LOG BOOK COURSE APPLICATION FORM:

Our Log Book Course is a 1 day in-class training course. Class times are from 8am - 5pm. Tuiton fee includes a manual and test. It is best to register early and pick up the manual and test before the day of the course to study and do the test at home.
Please select the course date you want to attend in the appropriate city. You must complete all fields on this form and submit the form. Incomplete application forms will not be processed. When you apply for one of our courses on-line, payment must accompany your application. Please provide your credit card information below. Application forms without payment cannot be processed. You may also print your completed form and mail it with a Certified Cheque, Bank Draft or Money Order, or fax the completed form to our office at 1-866-800-6837 and call us with your credit card info.
